Description:

Living Healthy Champlain provides a centralized program in the region to improve coordination of and access to self-management supports, programs and resources for people with chronic conditions. Our programming includes Powerful Tools For Caregivers, Living A Healthy Life with Chronic Conditions, Living A Healthy Life With Chronic Pain, Expert Led Health Literacy Webinars, Progress Over Pain, Whole Health, Whole Self, and One-One Peer Health Coaching.



All our programming is free to attend. Participants can attend in-person, online or via telephone. Our workshop calendar is updated regularly and can be viewed on our website. If you are unsure what program you or your patient or client might be best suited for, please feel free to call us directly. There is no waitlist for our programs.
