In-person program referrals generally require a visit by the Home & Community Care Coordinator to conduct a health assessment. In addition a referral visit to the main centre by client and/or caregiver(s) is required to provide information about program options, complete equired paperwork and help match client to the best suited day program. Home visit available by special request.
Sept 27, 2022 COVID Update
In-person programs with French speaking staff have re-opened.
French speaking staff available Tuesday & Thursday to support Francophones individuals to attend programs as a member of the larger inclusive and diverse groups.
* 2 programs offer a range of pace and social and fitness activities.
* 2 locations in Cambridge - transportation available for individuals living in Cambridge, North Dumfries, Kitchener, Waterloo and some surrounding areas
* PSW -personal care assistance available Thursdays for activities such as dressing, feeding, medication reminders, 1 person transfers, washroom assistance, personal hygiene care and mobility
* integrated - open to individuals with a wide variety of physical, mental, cognitive/developmental or neuro diversity
*offers opportunities for those with mild to moderate cognitive impairment to be engaged with the support of staff and volunteers
* opportunities for individuals with mildly limited social abilities to engage others in conversation & participate in activities
Program Types:
Golden Opportunities- Tuesday at Allan Reuter Centre- participants attending are independent and enjoy conversation. No PSW support available at this site location
Thursday at William E. Pautler Centre - offers a more relaxed, and easy pace ; PSW support available
This program has not re-opened: Living Well - Tuesdays at Fairview Mennonite Health & Wellness Centre - focus is on physical activity & fitness with 1.5 hours of physical exercise programs - no PSW support - plan to re-open September 2022.
Program is NOT suitable for individuals :
> requires use of mechanical or two person lifts to transfer
> with aggressive or exit seeking expressions;
> living with daily unscheduled bowel incontinence
> needing to sleep or lie down for a nap
> refusing or unable to engage and participate in activities outside their home
> has an age related dependency or frailty
> is living with a physical or mental impairment or chronic or degenerative health condition
> is experiencing depression, loneliness or social isolation
> requires assistance to remain independent and socially connected in the community
> lives with a caregiver or family member or is a senior caregiver who desires and needs some time away from their care giving role
AND
> lives in or around Cambridge or North Dumfries, Kitchener, Waterloo, Guelph and Wellington County or other surrounding communities
> will benefit from social interaction and stimulation in a supportive setting
> is willing and desires to attend and adult day service program
