Description:

This program is designed to help people live independently in their own apartments while providing access to 24/7 support. The program provides personal care support and homemaking services on a scheduled and unscheduled basis as needed. Individual care plans are designed to meet the needs of the client while optimizing their health and well-being as they live independently in the community.



Must reside in a Supportive Housing Building or be on a waitlist for housing at Benidickson Court, Gardner House and Parkview Apartments managed by: Kenora District Services Board (807) 468-5372



Eligibility determined by Home and Community Care Support Services Northwest: 1-800-626-5406 or 807-467-4757 to arrange for an assessment.

Cost to Client:
$125 monthly for Supportive Housing Services and access to 24/7 Support. This is over and above cost of rental apartment which is managed by Kenora District Services Board.
Eligibility Criteria

Minimum Age: 60 yrs
Criteria:
• 60 years of age or older
• Able to live independently and safely in their apartment between visits
• Agree with non-smoking legislation
• Must reside in a Supportive Housing Building or be on a waitlist for housing at Benidickson Court, Gardner House and Parkview Apartments managed by:
Kenora District Services Board (807) 468-5372

Supportive Housing CANNOT offer support to seniors with the following as we are NOT equipped, trained or staffed to do so:
• A history of alcohol and drug abuse causing a risk to self and others.
• Active psychological or emotional problems resulting in behaviour, which causes a risk to self or others.
• Total assistance due to bladder or bowel incontinence.
• Physical lifting or transferring in order to perform activities of daily living.
• Health problems requiring close professional supervision.
• Cognitive impairment resulting in inability to direct others to meet support needs or inability to meet their personal safety/security needs (due to minimal staffing and lack of appropriate locked exits and door alarms throughout building).
