Description:

The Telehomecare nursing program helps people living with conditions such as congestive heart failure and chronic obstructive pulmonary disease (COPD) achieve the best possible quality of life while learning to manage their condition, through remote monitoring and regular health coaching sessions in their own homes.



The client will receive guidance on checking their own blood pressure, weight, heart rate and pulse. With a touchscreen tablet provided through the program, they can send results, along with the answers to a few simple questions, to their Telehomecare nurse. If the readings fall outside of their normal range, the nurse is alerted and can respond as needed, and will also communicate directly with the client's primary care provider.



In addition to remote monitoring, the nurse provides weekly health coaching geared to the client's needs. This may include guidance regarding diet, exercise and other factors for optimal health.

Eligibility Criteria
Criteria:
People living with conditions such as congestive heart failure and chronic obstructive pulmonary disease (COPD). Assessment of needs and management of the respite periods of time will determine eligibility.
