Eligibility Criteria
Criteria:
-must have a referral from a physician or nurse practitioner and must be:-aged 65 years and older-aged 19 years and younger-a recipient of benefits through Ontario Works Program (OW)-a recipient of benefits through Ontario Disability Support Program (ODSP)-recently discharged as an inpatient of a hospital and in need of physiotherapy clinic services that are directly connected to the condition, illness or injury for which the person was admitted to the hospital
