Description:

The Community Alzheimer Program offers a variety of programs and services to support the needs of persons living with dementia, their families and care partners. Participants attending the program have the opportunity to engage in meaningful activities, develop relationships, and use their skills and talents in significant ways.



Current Service Offerings:
- Operating 5 days/week (Monday - Friday)
- Recreational and social activities on an individual or group basis.
- Nutritious lunch and snacks
- Assistance is available for those who require support with meals, personal care and medication (including diabetes)

Eligibility Criteria
Criteria:
  • Person has a diagnosis of dementia, and requires a specialized day program (i.e. cannot be supported in another Adult Day Program)
  • Physically and behaviourally stable, and does not require 1:1 ongoing support from staff.
  • Maximum one-person assist for care and transfers
  • Able to follow current Covid-19 protocols
  • Aware that the Community Alzheimer Program is unable to support smoking while in the day program.
  • Able to provide a local emergency contact who is able to pick-up the participant within 45-minutes of a call (i.e. becomes ill or cannot be safely supported in the day program).
  • Not currently living in a congregate care setting (retirement home/group home/etc.).
