How to Apply:
Please request consent to include an email address (client's or their alternate contact's) in the Caredove referral as they will automatically receive an email confirming that the client will be added to the waitlist list, and to provide day program information (i.e. wait time, location, cost, etc.). Please share client referral information in HPG.
**If also interested in the Community Alzheimer Program's Overnight Stay (ONS), a separate Caredove referral is required under the ONS Category.**
The program is NOT currently accepting referrals for clients who live in a congregate care setting (retirement home/group home/etc.).

Description:
The Community Alzheimer Program offers a variety of programs and services to support the needs of persons living with dementia, their families and care partners. Participants attending the program have the opportunity to engage in meaningful activities, develop relationships, and use their skills and talents in significant ways.
Current Service Offerings:
- Recreation and social activities on an individual or group basis.
- Nutritious lunch and snacks
- Assistance is available for those who require support with meals, personal care and medication (including diabetes)

Eligibility Criteria
Criteria:
- Person has a diagnosis of dementia, and requires a specialized day program (i.e. cannot be supported in another Adult Day Program).
- Physically and behaviourally stable, and does not require 1:1 ongoing support from staff.
- Able to transfer and receive care with a maximum one-person assist.
- Able to follow current Covid-19 protocols
- Aware that Sunnyside is a non-smoking campus, and that smoking cannot be supported while in the program.
- Able to provide an emergency contact who is available to pick the participant up from the Program within 45-minutes of a call, if the need arises (e.g. participant becomes ill or cannot be safely supported in the day program).
- Not currently living in a congregate care setting (retirement home/group home/etc.).
