Description:
Comprehensive assessment, treatment planning and case management support.
Short term psychotherapy and health teaching for seniors with complex mental health, addiction and behavioral issues. This includes:
- mood disorders (such as depression, mania) anxiety, psychosis, cognitive disorders, substance use;
- behavioral complications associated with neurodegenerative cognitive impairment ( e.g. Alzheimer's disease);
- chronic mental illness ( e.g. schizophrenia, bipolar disorder etc. ) complicated by age related decline in physical and mental health.
Note: Persons who are under age 65 years of age requiring specialized assessment and/or management as described above will be considered on an individual basis.

- Must have a primary care physician (If referred without primary care we will support the patient/family to source primary care through a variety of means).
