Description:

The My Way Home Program provides coordinated access to multiple Community Support Services with no financial barriers for a maximum 6-week period. The program targets seniors and adults with physical disabilities who are discharged from the hospital to home; present at the Emergency Department (ED) with needs related to social determinants of health; are at risk for hospital readmission, or as a preventive measure for emergency department visits.



The PATH (Priority Assistance to Transition Home) Program helps patients make the transition from hospital to home safely, smoothly and comfortably. When there are barriers to discharge the PATH and hospital team will work together to coordinate - implement - assist wherever possible, to ensure a successful stay at home

There is no cost to clients (applicable client costs covered by My Way Home Bundled Care Program)
Eligibility Criteria
Criteria:
  • MWH
    - At least age 55 or an adult with physical disabilities.
    - Client must be in need of more than one eligible service
    - In need of financial support for community support services
  • PATH
    - Age 55 and over
    - Able to direct their own care.
    - Medically stable and can be left unattended.
    - Be Independently mobile (unless family/caregiver is available to provide physical assistance).
    - Have been admitted (or discharged from) an inpatient unit or have presented at the Emergency Department
