Service Details
Description:

Free and reduced-cost health and support services for older adults and adults with physical disabilities across the City and County of Peterborough, Curve Lake First Nation, and Hiawatha First Nation. Referred individuals will be assessed and connected with the services that most closely align with their needs.



DAILY LIVING ASSISTANCE
Brokered Home Services – Matches client to vetted helpers for
paid support with home maintenance, light housekeeping, and yard work.
Grocery Delivery – Home delivery of groceries.
Meals Delivery – Hot and Frozen Meals on Wheels delivery with special dietary options.
Shopping Buddy – Volunteer accompaniment to support clients with shopping for groceries, clothing, gifts, and other items



TRANSPORTATION
Volunteer Drivers – Standard transportation for independently mobile clients.
Specialized Vans – Accessible transport for those with additional physical needs.
Access to Primary Care – PSW accompaniment for medical appointments.
Home at Last – PSW support for hospital-to-home transitions. Includes transportation, prescription pick-up, grocery shopping, and settling in services.



HEALTH & WELLNESS
Blood Pressure Clinic – Walk-in service for blood pressure checks.
Foot Care Clinic – Basic skin care and nail trimming.
Medical Equipment Rental – Short-term rentals of assistive devices.
Wellness Classes – Online and in-person wellness, exercise, and falls prevention classes suitable for adults 55+ of all abilities.



SAFETY MEASURES
Lifeline – Referral for discounted installation of a personal emergency response service, with options for fall detection and GPS tracking.
Telephone Reassurance – Scheduled safety check-ins.



SOCIAL ENGAGEMENT
Friendly Visiting – Companionship volunteers.
Social and Diner Events – Social dining experiences, craft groups, cards, and coffee groups.
