The Transitional Care Centre is located is a 27 bed unit, aiming to provide an environment promoting exceptional recovery and also simultaneously enriching the quality of life for individuals who are in need.
The Transitional Care Program is specifically for individuals who:
-Recuperating from surgical procedures or medical conditions
-Relieving caregivers who need a short break
-Waiting for admission into long term care home facility
