Description:

AccessCSS is a single point of access for community-based services for older adults, seniors and adults with disabilities living in the Champlain Region. Referrals made to community-based services that keep patients safe and at home.

Navigators refer to services and supports funded by Ontario Health that may include:

• Support & care at home
• Food and meal programs
• Home Help & Home Maintenance
• Day Programs, respite & caregiver support
• Safety and security programs
• Foot care
• Transportation
• Palliative Care
• Condition specific supports (dementia, acquired brain injury)
• Recreation, fitness & social activities
• Health Promotion & Education
• Navigation & Care Coordination

Referrals will be assessed and forwarded to the most appropriate community support service organization(s).
