Description:
Provide assessment, treatment planning, case management support, short term psychotherapy and health teaching for seniors with complex mental health, addiction and behavioral issues. This includes mood disorders (such as depression, mania) anxiety, psychosis, cognitive disorders, substance use; behavioral complications associated with neurodegenerative cognitive impairment ( e.g. Alzheimer's disease); chronic mental illness ( e.g. schizophrenia, bipolar disorder etc. ) complicated by age related decline in physical and mental health.
