Service Details
Description:
A community-based, 5-session, small group COPD education and exercise program.
Classroom education includes but is not limited to pathophysiology, medications, exercise, lung attack avoidance and management strategies and smoking cessation counselling (if applicable).
Program includes a pre-program assessment (done at SMGH Airway Clinic), 5 program sessions (done at 50 Bathurst Dr), and a post program assessment (done at SMGH Airway Clinic).
Airway Clinic staff will consider referrals to other SMGH Respiratory programs as appropriate

Eligibility Criteria
Criteria:
Patients must have a confirmed diagnosis or be at risk for development of COPD (please include a copy of the spirometry or PFT confirming diagnosis with referral)Stable cardiac status and stable comorbidities

Download and complete the attached "Airway clinic referral form", and fax to 519 749-6816. Please include results of spirometry or PFT test which have been completed within the past 12 months.
