Description:

15-hour supervised care and 24-hour emergency response to eligible clients in a designated building.
Supportive Housing provides assistance in the following areas:
Personal Support
- Showering/bed bath
- Mouth care
- Hair care
- Preventative skin care
- Assisting in transfers/positioning
- Dressing/undressing
- Assistance in eating
- Medication reminders
- Assistance with incontenence care
- Reassurance checks
- Essential Homemaking

Light housekeeping
- Laundry
- Meal planning
- Making beds

Cost to Client:

If you qualify for the Supportive Housing Program, there is no fee for the services offered.

Eligibility Criteria

Minimum Age: 65 yrs

Criteria:

Older adults and people with disabilities, who need physical, social, or emotional supports.An assessment will be completed to determine eligibility
