Description:

Traverse Independence offers outreach services throughout Waterloo Region and Wellington County for adults with acquired brain injuries. Support is provided to clients in their primary residence. Our outreach services assist clients with physical health, social and emotional well-being and integration into the community. The services are primarily aimed at improving clients' independent living skills.



Goal Attainment Program (GAP) - short term community support to accomplish specific goals. Community Facilitators work to support clients that don't require long term intervention. This support is able to be provided without a wait list and can be accommodated in program space or at an individuals home.



ABI in the Streets - specialized services for the homeless population.

Eligibility Criteria
Other Criteria:
Minimum Age: 16 yrs
Criteria:
- Have a valid Ontario Health Card- Have a brain injury diagnosed by a regulated health professional- Be medically stable- Demonstrate the desire to be involved in programs- Not be dangerous to themselves or others- Not have severe behavioural problems- Not have ongoing psychiatric or substance abuse issues
