Description:
The SMILE program makes it possible for frail elderly seniors, who are at risk of loss of independence, to remain in their own homes. The SMILE program provides access to services for frail elderly seniors (and their caregivers).

Services are arranged based on the needs of the senior and individualized care plans and budgets are developed to meet their needs. There is no set fee for any service authorized by the SMILE Program. Service providers are not employees of SMILE.
Eligibility Criteria
Criteria:
- Is the senior 65 years of age or older?
- Is the senior in need of assistance with daily living due to functional or cognitive impairment?
- Is the senior at risk for hospitalization or long-term care home admission?
- Is the caregiver unable to assist or is there a risk of caregiver burnout?
- Does the senior’s income fall below the financial threshold? Individual under $34,999 and couple under $49,999
