Assertive Community TreatmentProvided at homeProvides intensive support to people with serious mental illness who area struggling in several areas of life such as work, social relationships, housing, financial management and physical wellness. Delivered by a multidisciplinary team.
Chronic Disease Management Clinic
Individual clinical guidance for people diagnosed with a specific chronic disease such as arthritis, congestive heart failure, or diabetes. Requires clinical referral.
Complex Continuing Care
Hospital inpatient service for medically complex patients, often the result of a chronic or devastating illness. Care may include ventilation, special feeding, tracheotomy care, special pain management and palliative care.
Dietitian Consultation
Individualized nutrition counselling. Normally provided by a Registered Dietitian.
Geriatric Medicine Inpatient Service
Provides assessment and a treatment plan to improve mobility, address memory and thinking problems and manage chronic pain and everyday activities more independently and safely. Provided in hospital setting over several weeks.
Geriatric Psychiatry Clinic
Provides geriatric mental health assessment, diagnosis, counselling and treatment by a team who are experts in seniors' mental health.Typical concerns include confusion, anxiety, depression and psychosis.
Geriatric Psychiatry Inpatient Service
Provides psychiatric assessment and a treatment plan to address challenging cognitive, behavioural and medical issues of seniors. Provided in hospital setting over several weeks.

A service in honour of a deceased person.
Neurorehabilitation Clinic
Rehabilitation to aid in recovery from a nervous system injury. Treatment helps return the person to meaningful roles in life by regaining and relearning skills for everyday living.
Pharmacist Consultation
Guidance from a pharmacist to ensure medication is monitored and managed correctly.
Physiotherapy
Use of physical methods, including manipulation and exercise therapy aimed at restoring movement and balance after injury, disease or surgery.
Pulmonary Clinic
Services for people with respiratory disease such as COPD, cystic fibrosis aimed at improving the quality of life and supporting families. Often involves exercise and drug treatment.
Speech-Language Rehabilitation Clinic
Identification, evaluation, and treatment of acquired communication disorders such as difficulty understanding or producing speech and swallowing. (Includes aphasia)

Spiritual care providers help to integrate spiritual and religious aspects of care, as defined by the individual, into the care of the whole person - body, mind and spirit.
