Community Care Consultation Provide consultation to primary care practitioners and other clinicians regarding home and community care options available in the community.

Community Centre Facilitates activities in a supported setting such as recreation, fitness, social, educational programs and events. Specialized programs in post-rehabilitation, chronic disease management, and nutrition education may also be available. Usually membership based.

Foot Care Clinic Provides nursing foot care and advice for clients to address common foot problems such as thickened and fungal nails, corns, calluses, bunions.

Friendly Visiting Provided at homeOffers no charge visiting companionship such as conversation, reading, games, or crafts, while providing a safety check. The contact may be face-to-face at the client's home or through a phone call.
