Care CoordinationProvided at homeIntegrates health and social services by facilitating referrals and access to service across agencies. Supports care by providing follow-up after hospitalization or emergency room visits, or after specialist assessments.
Fitness Group
Exercise sessions where participants develop physical fitness and/or skills within a defined sport. May be either beginners or advanced.
Health Education Classes
Provides education to participants to make choices that may improve wellness or reduce risks of disease. May focus on specific diseases or specific lifestyle issues (e.g. healthy eating).

Overdose Prevention Site
A safe, hygienic environment for people to inject pre-obtained drugs under the supervision of staff, with a goal of reducing drug overdoses and infection. Connects people with other services.
Seniors Gentle Exercise Class
Provides group exercise classes, typically one hour in length, to improve strength and balance. Education provided to reduce risk of falls.
Social Activity
Social programs that help keep people engaged with their friends or community. Activities are usually 1-3 hours and are often recreational. Programs may be offered virtually.
Support Group
In-person meetings where members provide each other with help for a particular life challenge. Often involves relating personal experiences and providing sympathetic understanding. May be clinician or volunteer-led.
