Community Care Peterborough

This service is for hospital inpatients only.

Service Details

Description:

Home First promotes safe and timely care, services, and supports to meet the needs of patients and their families in the most appropriate setting. It enables all patients - particularly seniors - to return to their home from hospital safely and to remain at home, preventing a hospital re-admission.

Community Care Peterborough’s Home First Care Navigator works as part of an interdisciplinary team at PRHC. The Care Navigator collaborates with PRHC and Home and Community Care staff to develop individualized care plans for patients and their families to optimize well-being in support of safe and timely discharge from hospital.

Patients who are assessed by the Care Navigator may be eligible to receive short-term assistance with services such as meals, transportation, respite, Lifeline emergency alarms, and the purchase or rental of mobility equipment.

Referrals to Home First are made through PRHC and Home and Community Care Support Services only.
