The Alzheimer's Day Program is aimed at adults who want to stay in the community in their own homes or with their families, and are coping with the specific effects of Alzheimer's Disease, or other forms of dementia. Participants take part in a range of arts, cultural, gardening, cooking, and strength training. The program focuses on the abilities of our participants despite the complicated symptoms that can accompany dementia. The day program also provides caregiver relief, education for families and individuals, support and advocacy. Here is an example of our Calendar: http://www.sjhcg.ca/alzeimers-day-program
Program is NOT suitable for individuals:
-Smoke and need to leave the program space during program hours
- who require two person transfers
- who require the use of a mechanical lift
- who require constant 1:1 support
- who require feeding assistance
- Physically and behaviourly stable and does not require 1:1 ongoing support from staff.
- Be able to independently feed themselves.
- Can/ Will the individual participate in group based programming?
- Person has a diagnosis of dementia and requires a specialized day program.
- Able to transfer and receive care with a maximum 1 person assist (unable to support the use of transfer boards).
- Aware that SJHCG is a smoke free facility, and that smoking cannot be supported while in the day program.
- Able to provide an emergency contact who is able to pick up the participant from the program within 45 minutes of a call, if the need arises (ie. participant becomes ill, or can not be safely supported in the program).
- Able to provide an email address for the participant or the caregiver to receive the referral confirmation (no longer able to provide telephone confirmation).
