Description:
LEGHO is a 4-6 week program that offers navigation and community support services for people who have been recently discharged from hospital. This program covers the cost for people to receive services and support for free. Services may include frozen Meals on Wheels, transportation for medical appointments, senior support worker (e.g., personal support, light housekeeping, laundry), home safety assessment, phone check-ins, etc.

Eligibility Criteria
Criteria:
- Recently discharged from a hospital.
- The client is 60+ or an adult who is at risk of visiting ED again in the next two months.
- The client is ALC, medically stable and ambulatory.
- The client is not currently living in LTC or retirement home.
- This referral is generally not for mental health, addiction and substance use support.
