Service Details
Description:

The ABI day program provides social and educational activities, and community outings. It also provides community case management to clients and families, overnight respite, and consultation with a physiatrist.



Participants bring their lunch, and the program provides light snacks.



Supervised and supported social and recreational activities for seniors who are physically frail or have challenges, at a location outside their home. Programs include meals, transportation to the program, and some personal care.

Cost to Client:
The cost of the program is covered for clients with a valid OHIP card issued by the Ministry of Health.

Other Criteria:
Minimum Age: 18 yrs
Criteria:
- At least 18 years old- Have moderate to severe acquired brain injury- Medically stable- Currently living in, or preparing to return to, the community- Have a family physician
