Provides assistance with personal care, meal preparation, essential housekeeping, laundry services, medication support, and escorts to appointments. Psychogeriatric case management services include assistance navigating the health care and social services systems, and in finding and accessing any services an individual resident may require. St. Anne’s residents also enjoy a program of activation and socializing through planned social activities, excursions and special events.
